Founded in 1995 by CEO Leonard Osser with inventor and friend Dr. Mark Hochman, the company has researched, developed and brought to market a full suite of award-winning, FDA-approved, drug delivery injection instruments used by medical and dental practitioners in over 25 countries throughout North America, Europe, Asia and Africa. With the corporate office based in Livingston, New Jersey, a manufacturing plant in Chicago, Illinois, and distributors throughout US, Europe, China, and Asia, the firm produces injection technologies geared towards both GPs and specialists.

Cultrara, RDH, director of education says that as a long-term goal, “We want every doctor that uses our instrument to give anesthesia in a much more beneficial and accurate way than ever before and to become syringe-free.” Noting that Milestone’s “All Injection System” is different than other  injection products in the dental field, she explains, “Most products in the dental industry have evolved, but the dental syringe has stayed the same. Although most dentists give a pretty good injection, as patients we have all experienced the discomfort of a dental injection and have experienced the after-effects of anesthesia, like being numb for hours afterwards.”

In 1997, Milestone introduced the first generation of its Computer-Controlled Local Anesthetic Delivery (C-CLAD™) system, known as The Wand® (and later rebranded as CompuDent®), that was composed of two basic parts, the delivery instrument and the single-use, disposable Wand® handpiece.  According to Milestone Scientific, this represented the most successful launch in the history of small equipment sales in U.S. dentistry, with more than 18,000 CompuDent instruments sold within 4-1/2 months of its market introduction.

Currently, the award-winning Wand®  ”All Injection System” instrument combines patented, state-of-the-art computer-controlled injection technology with the dentist’s training to achieve more precise injections, improved drug delivery and materially enhanced patient comfort levels. Utilizing Milestone’s proprietary DPS Dynamic Pressure Sensing technology, audible and visual real-time “feedback” is provided to the clinician to accurately guide and locate a specific injection site by continuously measuring exit-pressure at the tip of the needle.

The Wand® handpiece, used in conjunction with the instrument is ergonomically designed with a pen-like grasp, and feather-like weight to produce tactile control for every injection.

The Wand handpiece allows bi-directional rotation during injection, which prevents needle deflection that occurs with a traditional syringe.   A straighter path results in a more accurate injection.

The Wand® “All Injection System” is designed for precise control and monitoring of all critical variables during drug delivery, including:

• a  true “painless” experience for all injections
• eliminates disruptive injection behavior
• site specific targeting
• controlled needle exit-pressure
• precise flow rate and drug volumes
• patient treatment documentation
• superior ergonomics
• elimination of needle deflection
• advanced tactile needle control
• precision fluid metering.

To spread the news about this innovative “All Injection System,” the firm has developed a strong training and support team. Cultrara says, “We don’t just drop our instrument at an office and say, ‘Here you go--figure it out.’ We pride ourselves on providing whatever the customer needs, with the same level of support throughout every department. If we say we are going to call back at a certain time, we really do.”

Director of International and Professional Relations Dr. Eugene Casagrande, travels throughout the US and worldwide with Director of Domestic Distribution Marvin Terrell and Director of International Distribution  Dale Johnson to train the distributors and representatives in the proper use of the products. Dr. Casagrande is past President of the California State Board of Dentistry and the Los Angeles Dental Society, is a Fellow of the American and International Colleges of Dentists and has served on the faculty of the University of Southern California, School of Dentistry.  Both Terrell and Johnson are 25-year veterans of the pharmaceutical and dental industry.

Milestone also touts a training and support team of 42 clinical hygienists and additional dental assistants in the field that visits distributors, single-doctor offices, and group practices. In many states, hygienists are certified to give anesthesia and also need to be trained in the product. “We work with the entire staff,” says Cultrara. “We work with the assistants because they assure that the instrument is set up and maintained properly.  We work with the front desk because patients will be asking questions or calling on the phone to find out what technology the doctor uses.”

For times when the doctor is too busy for in-office training or has a quick question, remote training is available. “We can train during a lunch time, or the doctor can quickly connect me with one of the hygienists in the office,” says Cultrara. “It allows us to see each another so I can see what he/she is doing. Probably one of the biggest accomplishments we have made is regarding how training and support has evolved for this product.” She sums up, “Our ultimate goal is to provide doctors who use our product with a much more accurate, comfortable, and precise way to give anesthesia.”
